The LPN must display a commitment to the missions, values, and vision of Hattie Larlham. The LPN is responsible for participating in the individual plan development and implementation of services in assigned homes including, but not limited to, the overall health & safety of clients. Monitors, assesses, provides and coordinates health care services to for the people served in the ICF/IDD group homes as outlined in their plans of care and IHP. The LPN is responsible to ensure that all applicable standards are met and maintained at all times. This includes, but is not limited to: Ohio Department of Health (Medicaid), DODD (Licensure).

Full-Time, 2nd Shift, 3pm-11pm, every other weekend.  Must be flexible to meet the needs of the persons served. Will work nights, evenings, days, weekends, and holidays as scheduled. May be called to work mandatories if the facility is medically short staffed to meet the needs of the persons served. May be required to drive the group home van. May work with persons served with severe behavior disorders. Will work with both handicapped males and females.

•            Licensed Practical Nursing degree from an accredited school.

•            Must maintain current licensure as an LPN in the State of Ohio.

•            Must maintain current CPR certification.

•            Previous direct care or nursing experience preferred, but not required.

•            Must be motivated, hardworking, patient, and trustworthy.

•            Willing to learn rules and regulations to provide care that is based on best practice   standards.

•            Displays and accepts responsibility of their actions, the actions of their staff, feedback from others and is fiscally accountable for areas of the budget within their control
